SHINGLES VACCINE CLINIC
If you are 60 years and over, you may want to get a Shingles Shot. We will be hosting the Pharmacy at Safeway on Tuesday, February 23rd in the Phoenix Room (A-9 & A-10) from 8 a.m. – 12 (noon). Forms are available at the CW Resource Center. Please submit the completed form by mail, fax or hand deliver, no later than February 16th to: Safeway Pharmacy, 4970 S. Alma School Road, Chandler 85248, (480) 883-0260 or FAX (480) 883-0270. Once they’ve received your form, they will verify insurance coverage and call to inform you of your co-payment and schedule your appointment. If you’re not covered by their accepted insurance, Safeway will give you a receipt to submit to your insurance for reimbursement.
